Abstract. Development of information and communication technology at this time from time to time has progressed very rapidly. This has been used by several government and private agencies to make the process of sending and receiving information faster and more efficient. The process of submitting and receiving insurance documents by several insurance companies still uses a simple method. For example, customer insurance filing documents submitted by the agent via email to head office for further processing. But in reality, there are still many documents that do not meet the requirements for filing documents. Of course this will result in a delay in the process of submitting insurance services. The result of this research is a-based application web using the PHP programming language (Hypertext Preprocessor) and MySQL as data storage in the database. The author uses several data collection methods, such as observation, interviews and literature studies.
